YSEP ENROLLMENT AND PARTICIPATION PARENTAL CONSENT

This document serves as the Parental Consent Form for the learner's participation to the program/s offered by Studentship Educational Innovations Philippines, OPC (Studentship Philippines) such as the Young Scientists Enrichment Program (YSEP). By agreeing to this document, the parent/guardian confirms that they have read the document in full and that they agree to the policies prescribed herein. This must be strictly accomplished by the parent/guardian only. Only learners whose parents/guardians have agreed to the Consent Form may access the site and proceed with the program.

STUDENTSHIP PHILIPPINES ENROLLMENT AND PARTICIPATION PARENTAL CONSENT

I hereby agree and approve to my child’s enrollment and participation to the learning programs and other relative services provided by the school and Studentship Educational Innovations Philippines, OPC (Studentship Philippines).

I hereby consent to the school and Studentship Philippines the collection, use, and/or disclosure of information about my child through the course registration process and other manual and/or electronic procedures.

 I understand that my child is registering for and participating in an online or hybrid (combination of online and in-person) programs, and that the information collected is needed for both registration and progress reporting.

I understand that my child, his/her peers, and the school may be featured in print publications, online publications, presentations, websites, and social media. Studentship Philippines and the school have my permission to use my or my child/children's image, likeness and sound of my/their voice as recorded on audio, photograph or video tape for these purposes. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

 It is understood that my son/daughter shall abide by the rules and regulations that may be imposed by the school and Studentship Philippines. It is further understood that I fully agree to waive any responsibility on the part of the school, Studentship Philippines and staff in case any untoward incident happens to my son/daughter during the entire duration of the program.

 IMPORTANT NOTES ON YSEP:

  • Participation to YSEP LITE is free.
  • Participation to YSEP PLUS requires minimal fee.
  • Participation to YSEP PLUS is OPTIONAL and VOLUNTARY.
  • Parents are informed and acknowledge that enrollment of their child to YSEP PLUS is a direct transaction between the parents and Studentship Philippines (similar to that of availing the services of a tutorial center outside of and external to the school); thus, payments are transacted directly with a Studentship Philippines representative and/or may be made thru GCash and the Studentship Store (online).
  • YSEP PLUS will be conducted at a time, date and venue that adheres to the no disruption of classes policy and will thus be identified by and at the discretion of the school.
